Female hair thinning follows a different pattern than male hair loss — diffuse across the crown rather than receding from the hairline. The mechanism is the same at the follicle level, but the distribution, the hormonal drivers, and the treatment response all differ.

Follicular miniaturization and the microenvironment that drives it

Each hair follicle contains a dermal papilla — a cluster of cells that receives signals from hormones, the blood supply, and the surrounding scalp environment, and uses those signals to direct hair growth. When the microenvironment deteriorates, papilla signaling weakens. The follicle responds by producing progressively thinner, shorter hair shafts — a process called follicular miniaturization.

Three factors drive this deterioration in women: declining estrogen (which normally protects follicle function and prolongs the growth phase), reduced scalp blood supply (which limits nutrient and oxygen delivery to the dermal papilla), and DHT sensitivity in genetically susceptible follicles (which shortens the anagen growth cycle over successive rounds).
